The Modality Ladder

Choosing Your Modalities

Most scan centres start with one or two modalities and add more as volumes grow. Each step up changes the space, shielding, approvals and staff you need.

  1. Step 01

    Ultrasound (USG)

A compact room with no ionising radiation, making it a common starting point. It needs PCPNDT registration and a qualified sonologist or radiologist to perform scans.

  2. Step 02

    X-Ray / Digital Radiography (DR)

A shielded room with a separate operator position. It needs AERB registration and a designated Radiation Safety Officer.

  3. Step 03

    CT

A larger shielded gantry room with its own control room, stable power and air-conditioning. The room layout must be approved by AERB before a licence is issued.

  4. Step 04

    MRI

No ionising radiation, but the room needs RF shielding, magnetic-safety zoning and careful structural and cooling planning. It is usually the largest investment of the four.

The Process

How a Scan Centre Project Runs: Six Phases

  1. Phase 01

    Catchment & Modality Mix

Who refers in your area and which scans they send out of town. That decides which modalities earn their place on day one and which can wait for volume.

  2. Phase 02

    Layout, Shielding & Patient Flow

Room sizes and positions worked out around the shielding each modality needs, with the control position, changing space and waiting area planned so patients and staff do not cross.

  3. Phase 03

    Equipment Selection & Procurement

Machines matched to expected daily scans rather than to a brochure, with installation, calibration and handover managed through to first use.

  4. Phase 04

    AERB, PCPNDT & Licensing

The approvals below, prepared and filed in the order they have to happen — layout approval before installation, licence before the first patient.

  5. Phase 05

    Radiologist Cover & Reporting

Deciding on-site, tele-reporting or a mix, and setting up the workflow so a scan leaves the machine and comes back as a signed report.

  6. Phase 06

    Launch & Referral Building

Staff training and commissioning, then the doctor outreach and local marketing that bring the first referrals through the door.

Before You Scan

Regulatory Approvals for Imaging

Imaging has approvals that must be in place before you scan your first patient. Requirements vary by state and by the equipment you install, so treat this as a starting checklist.

  1. 01

AERB registration or licence for X-ray and CT, including approval of the room layout

  2. 02

Radiation shielding designed to the approved layout and verified before use

  3. 03

A designated Radiation Safety Officer (RSO) for X-ray and CT

  4. 04

PCPNDT registration for ultrasound, with the mandatory notice displayed

  5. 05

Registration under the Clinical Establishments Act (or your state's equivalent)

  6. 06

Biomedical waste authorisation, fire NOC and trade licence

Which approvals apply depends on the modalities you install. We confirm the list for your centre and file them in the order they have to happen.

Two Early Decisions

Who reports the scans, and what you buy

Radiologist Availability & Reporting

Equipment is only half the plan: every scan needs a radiologist to report it. Decide early whether reports will come from an on-site radiologist, tele-reporting, or a mix of both. Blood Test Consultancy works with a network of 150+ radiologists and pathologists across India, which helps when planning reporting cover for a new centre.

Refurbished or new equipment?

Refurbished machines lower the entry cost and can make sense where volumes are still building. New machines carry full warranty, current software and a longer service life. Whichever route you take, check that spares and service cover are available locally and that the model can be type-approved for the licence you need — those two things matter more than the sticker price.

Complete Scan & Imaging Centre Setup

From the first shielding drawing to the first signed report.

Scan centres we have set up include Affiniti Scan Centre. See more of our work on the Projects page, or read about combining imaging with a lab on our Diagnostic Centre page.

Imaging Rooms We Have Set Up

Modality rooms planned around the equipment they house — shielding, service routes and compliance signage handled before handover.

Digital X-ray room with ceiling-mounted tube and Horizon table
Digital X-Ray Room

A digital radiography room with a floor-mounted column, motorised table and a separate control position. Room shielding and cable routing are planned before the unit is installed.

Ultrasound scanning room with colour Doppler machine, couch and PNDT notice
Ultrasound Suite

A colour Doppler scanner set beside a curtained couch, with the mandatory PNDT Act notice displayed. Compliance signage goes up as part of the handover, not after.

Cardiac stress testing room with treadmill and TMT recording workstation
Cardiac Stress Testing

A TMT setup with treadmill, recording workstation and reporting printer in one room — a low-footprint service line that complements imaging and pays back quickly.
